Dental Care for Grade 7

Dental Care is very important for all children.  It is Mandated by Pennsylvania School Code that ALL students in Grade 7 have a report of dental care completed for their school health record.

Options:

1.  Continue with your already scheduled Dental examination with your Private Family Dentist (form available on District website under "Forms").  Have the dentist complete the required form after the exam.  The form can be handed in directly to the school nurse, faxed to 717-755-5914, or mailed to Mrs. Kopelic, School Nurse,  1950 North Hills Road, York, PA. 17406

2.  PA SMILE PROGRAM performs exams, fluoride, sealants, and x-rays when needed during school hours.  Registration is required for this service. It is open to all insured and non insured students. Children will not be seen without proper registration.  Please contact your school nurse for a form.  Forms are also available by clicking on the Forms & Documents tab.
