All student-athletes at Central York School District are required to have a SportsWare account. This is the ONLY way to obtain the proper documents required for athletic physicals and participation requirements. SportsWare is an online platform used by the Central York Athletic Department to simplify the process of disseminating information and housing each student-athlete’s medical record. This account will remain active until the student-athlete graduates from CYHS. Please ensure that you retain the log-in and password information as this account will need to be accessed for EACH athletic season your child participates. Registering for SportsWare ensures that you will receive, via email, all pertinent information regarding athletic physicals and information specific to this process. It is HIGHLY RECOMMENDED that the email on record belong to a PARENT/GUARDIAN and NOT the student.  A PARENT/GUARDIAN should be the primary source of entering data into the account.  Please make sure the email used is checked regularly, this is how we communicate information to you.

  • You may access the instructions by visiting the Central York Athletics Web Page:
    • Please understand that your request to join SportsWare needs to be approved before you may begin the process of entering data. You will receive a return email from SportsWare with further instructions.

  • Enter all data that is required to be completed. All fields marked with a red asterisk (*) must have an entry.
  • Ensure that you have picked a Sport.
    • Fall Sports = SPORT 1
    • Winter Sports = SPORT 2
    • Spring Sports = SPORT 3
  • Enter “N/A” for any field that does not apply
  • Please pay attention to any messages that appear when you save and exit.
  • This informs you about any missing items.

  • You will have up to five forms to complete. The program uses electronic signatures to eliminate the need to print the entire document. Print ONLY the forms that are identified “PRINT”. These forms will include:
    • Part 1 PIAA CIPPE Sec. 1-4
    • Part 2 PIAA CIPPE Sec. 5-6 (Print for Physical)
    • Impact testing Consent Form
    • Return to Play Consent Form and Instructions
    • Recertification Form (Winter and Spring Seasons ONLY- these forms are released later in the year, you will receive an email about this)

  • Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, at any PIAA member school in any school year, the student is required to (1) complete a Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE); and (2) have the appropriate person(s) complete the first six Sections of the CIPPE Form.  Upon completion of Sections 1 and 2 by the parent/guardian; Sections 3, 4, and 5 by the student and parent/guardian; and Section 6 by an Authorized Medical Examiner (AME), those Sections must be turned in to the Athletic Training Staff, of the student's school for retention by the school.  The CIPPE may not be authorized earlier than June 1, 2020 and shall be effective, regardless of when performed during the school year, until May 31, 2021 or the conclusion of the spring sports season.

  • Central York Athletics was unable to host physicals this year for winter athletes. However we have made accommodations for athletes to obtain them for $15 with no appointment needed at Wellspan or with an appointment at OSS. Trainers and coaches have this information if needed.
